1.Nature of Telehealth Care
Telehealth uses electronic communications (secure messaging, video, and asynchronous review of forms, photos, and lab results) to deliver clinical care without an in-person visit. Your Clinician may be located in a different city or state than you.
2.Benefits
Benefits include improved access to care, convenience, privacy, reduced travel, and the ability to communicate with a licensed Clinician from your home.
3.Risks & Limitations
Limitations include: no in-person physical examination; potential for technology failures, delays, or interruptions; possibility that information transmitted may be insufficient for clinical decision making; risk of unauthorized access despite security measures; and the possibility that your Clinician may determine telehealth is not appropriate for your condition and refer you for in-person care.
4.Alternatives
You may seek care in person from a local clinician at any time. Telehealth is one option, not the only one.
5.Voluntary Participation
Your participation is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care. If you withdraw consent, we will stop providing telehealth services to you.
6.Records & Privacy
Your medical record is maintained securely and is governed by our HIPAA Notice of Privacy Practices and Privacy Policy. You have the right to inspect and request a copy of your record.
7.Communications
Messages between you and your care team are part of your medical record. Replies are not in real time. Do not use messaging for emergencies.
8.Consent to Prescription Treatment
If a Clinician determines a prescription is appropriate, you consent to having that prescription sent to a partner pharmacy and shipped to the address you provide. You have read or will read the medication’s safety information, including any boxed warning, and you understand that some prescribed medications may be compounded and prepared by a state-licensed 503A compounding pharmacy (see our Medical Disclaimer). You agree to follow your Clinician’s instructions and to report side effects promptly.
9.Off-Label Use
Some medications a Clinician may prescribe are used off-label — that is, for a use, dose, patient population, or formulation that the U.S. Food and Drug Administration (FDA) has not specifically approved. For example, certain GLP-1 receptor agonists and other therapies may be prescribed off-label for weight management or wellness goals, and compounded formulations are not FDA-approved drug products. Prescribing a legally marketed medication off-label is lawful and common in clinical practice when supported by a Clinician’s medical judgment. You understand that off-label use may involve risks and benefits that are not described in FDA-approved labeling, and you consent to off-label treatment when your Clinician determines it is appropriate and has discussed it with you.
10.Pharmacy of Your Choice
If a prescription is issued, it may be routed to a partner pharmacy for your convenience. You have the right to have any valid prescription sent to the licensed pharmacy of your choice. To request this, contact us at hello@zepym.com or +1 (877) 959-3796 with the pharmacy’s name, address, and phone number, and we will forward your prescription accordingly.
11.Insurance & Payment
Telehealth services and medications obtained through the Service are cash-pay and are not billed to or reimbursable by Medicare, Medicaid, or any other federal, state, or private insurance program. You agree not to submit, and not to ask anyone to submit on your behalf, a claim for these services or medications to Medicare or any other payer.
12.Emergency Care
Telehealth is not appropriate for medical emergencies. Call 911 or go to the nearest emergency room for chest pain, suicidal thoughts, severe allergic reactions, signs of stroke, or any life-threatening symptom.
13.Acknowledgement
By completing intake and continuing to use the Service, you acknowledge that you have read, understood, and agree to this Informed Consent.
